Why is my competitor showing up in ChatGPT and I'm not?

Almost never because they have more reviews or spend more on ads — those win the map pack, not the AI's written answer. It's because their information is published in a way the AI can read and trust for the exact question a patient asked, and yours isn't. ChatGPT builds its answer from sources it can confidently pull from; your competitor simply gave it something to pull. The upside: it's a specific, fixable gap, not a popularity contest.

It's counterintuitive, but reviews and ad budget don't decide the AI answer. We've seen clinics with huge review counts left out of the AI's cost answer in their own city while smaller clinics get named. Reviews and ads move Google's map pack; the AI's written recommendation is a separate game, decided by what the model can extract and corroborate.

What your competitor most likely has is a real, readable answer to the exact question — and at least one other source that agrees with it — so the AI can name them with confidence. It usually isn't a bigger brand or more money; it's that they (or someone they hired) made their key facts easy for the AI to use, and most clinics simply haven't.

It can feel unfair, but it's actually good news: a gap you can close beats a popularity contest you can't. AI tends to name only a small handful per city, and being established as one of them compounds — so the earlier you close the gap, the harder you are to displace.
